چکیده

Purpose/Objective(s) Recurrence rate after salvage radiotherapy (sRT) is heterogeneous and additional prognostic markers are needed to improve risk-stratification guide personalized treatment. We evaluated the association of maximum standardized uptake value (SUVmax) in positron-emission tomography targeting prostate-specific-membrane-antigen (PSMA-PET) prior sRT on biochemical recurrence free survival (BRFS) a large multicenter cohort. Materials/Methods Patients who underwent 68Ga-PSMA11-PET radical prostatectomy (RP) due or PSA-persistence were enrolled four high-volume centers this retrospective study. Only patients with PET-positive local (LR) and/or nodal (NR) within pelvis included. treated intensity-modulated-sRT prostatic fossa elective lymphatics case disease. Dose escalation was delivered LR NR. Androgen deprivation therapy administered at discretion treating physician.LR NR manually delineated using SUVmax extracted for Cox-regression performed analyze impact clinical parameters SUVmax-derived values BRFS, defined as serum prostate-specific-antigen >0.2 ng/ml above post-sRT-nadir without initiation therapies death any cause. Results 235 median follow up (FU) 24 months included final The presence associated favorable BRFS (p=0.016). Presence unfavorable (p=0.007). While there trend ≥ (p=0.071), 75%quartile significantly (p=0.022, HR: 2.1, 95%CI 1.1-4.6). not BRFS. stayed significant multivariate analysis (p=0.030). Sensitivity had FU >12 (n=197) confirmed these results. Conclusion In recurrence/persistence RP, PSMA-expressing pelvic lymph nodes PET less salvage-radiotherapy. non-invasive biomarker can prognosticate outcome undergoing confined PSMA-PET.

Salvage radiotherapy for recurrent prostate cancer after radical prostatectomy.

CONTEXT Salvage radiotherapy may potentially cure patients with disease recurrence after radical prostatectomy, but previous evidence has suggested that it is ineffective in patients at the highest risk of metastatic disease progression. OBJECTIVE To delineate patients who may benefit from salvage radiotherapy for prostate cancer recurrence by identifying variables associated with a durable r...

[Prostate cancer recurrence after radical prostatectomy and salvage radiotherapy].

OBJECTIVE To evaluate the efficacy of the radiotherapy to prostatic bed in patients with biochemical recurrence for prostate cancer after radical prostatectomy. MATERIAL AND METHODS We analyse the results of 292 patients underwent radical prostatectomy for localized prostate cancer T1-T2 between January 1992 and June 2003, with an average folow-up of 36 months (range 6 months to 12 years). We...

Salvage External Beam Radiotherapy for Prostate Cancer After Radical Prostatectomy

Prostate cancer is the second most common cause of cancer death in American men, with 192,289 new cases and 27,360 deaths expected in 2009.[1] While radical prostatectomy provides excellent control for clinically localized disease, approximately one-third of patients undergoing surgery will have positive surgical margins and another 9% will have seminal vesicle invasion.[2-5] Around one-third o...

Salvage conformal radiotherapy for biochemical recurrent prostate cancer after radical prostatectomy.

OBJECTIVE Assess the results of salvage conformal radiotherapy in patients with biochemical failure after radical prostatectomy and identify prognostic factors for biochemical recurrence and toxicity of the treatment. MATERIALS AND METHODS From June 1998 to November 2001, 35 patients were submitted to conformal radiotherapy for PSA > or = 0.2 ng/mL in progression after radical prostatectomy a...
